  • Please copy the following into the body of your message and answer the questions:

  a.        Patient’s name?
  b.        How many boxes would you like for each eye?
  c.        Will you be using any insurance benefits for this purchase?
  d.        Daytime phone number?
  e.        If applicable, please type the address where you would like them shipped
            (no PO Boxes).
  • Put “Contact lens order” in the subject line.

  • All orders will be shipped to the office unless you request otherwise.  

  • If you would like them mailed directly to you, a $6.95 shipping charge will be
    applied.  If you purchase an annual supply of lenses, this charge will be waived.   

  • We will need payment in full to place the order. Trina will contact you for a credit
    card number, or you can stop in and pay at the office.
