  • Please copy the following into the body of your message and answer the questions:

    a.        Patient’s name?
    b.        How many boxes would you like for each eye?
    c.        Will you be using any insurance benefits for this purchase?
    d.        Daytime phone number?
    e.        If applicable, please type the address where you would like them shipped
              (no PO Boxes).
  • Put “Contact lens order” in the subject line.

  • All orders will be shipped to the office unless you request otherwise.  All lenses
    shipped to the office can be paid for at pick-up.  

  • If you would like them mailed directly to you, a $6.95 shipping charge will be
    applied.  We will need payment in full to place the order. Trina will contact you for
    a credit card number, or you can stop in and pay with cash or check.
